Key Responsibilities

• before care is initiated.

• Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.

• Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and obtains orders for changes in the plan of treatment.

• Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures, and techniques to clients.

Requirements

• Focus on great care without worrying about on-call rotations.

• Position Highlights: Accurately assesses clients at admission and other required time points and utilizes appropriate standardized documentation.

• Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.

• If therapy only case, informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated.
