About the Role

Join BAYADA Hospice as a compassionate Nurse Practitioner providing home-based hospice and palliative care in Concord, New Hampshire. Prior hospice or home care experience preferred but not required.

Key Responsibilities

• Conduct face-to-face hospice recertification visits.
• Monitor client conditions and report changes.
• Execute and document physician orders.
• Perform client assessments as needed.

Requirements

• Current Nurse Practitioner license in New Hampshire.
• Minimum one year recent clinical experience.
• Strong interpersonal and computer skills.
