Key Responsibilities

and Responsibilities Assumes the responsibility for the on-going interdisciplinary and individualized plan of care in partnership with the patient, family/representative and/or caregiver(s).

Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.

Evaluates home environment.

Provides patient, family, and/or caregiver counseling, education, and progress summary based on the attainment of goals.

Candidates must possess and maintain licensure to practice Physical Therapy in Washington State and have graduated from an accredited program with at least a baccalaureate or doctorate degree in physical therapy. A valid driver's license, personal automobile with insurance, and active BLS CPR certification are also mandatory requirements.
