Physical Therapist- Home Health Visits

About the Role

BAYADA Home Health Care seeks an experienced Physical Therapist for full-time or part-time home health visits in Montgomery County, Maryland. You will provide rehabilitative care to adult and geriatric clients in their homes.

Key Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, balance, strength, and safety assessments.
• Implement wound prevention and repair interventions.
• Develop and revise individualized care plans.
• Educate clients and families on rehabilitative care.
• Document care management and services accurately.

Requirements

• Current Maryland Physical Therapist license.
• Minimum one year recent clinical experience.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently with strong organizational skills.
• Computer proficiency; EMR experience preferred.
