On -Call Weekday/Weekend Registered Nurse - Home Health Visits

About the Role

This is an on-call Registered Nurse position providing skilled nursing and rehabilitative care to adult clients in their homes throughout Howard and Carroll Counties, Maryland. The role supports patients to remain safely at home through compassionate care.

Key Responsibilities

• Follow designated care plans based on patient needs.
• Conduct home visits within assigned geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor and report changes in patient conditions.
• Execute and document doctors' orders accurately.
• Perform client assessments and case management.
• Document observations and interventions using electronic tablets.

Requirements

• Current Maryland Registered Nurse license.
• Minimum one year recent clinical (medical/surgical) experience.
• Graduation from accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and computer skills; EMR experience preferred.
• Ability to travel to assigned cases.

Work Schedule

• On-call shifts from 5:00 pm to 8:30 am Monday through Friday mornings.
• Weekend on-call from 5:00 pm Friday to 8:30 am Monday.
• Evening and overnight hours required.
• Weekend coverage required.
