Join Luminis Health in Annapolis, MD as a Mental Health Clinician in a partial hospitalization outpatient setting. Provide brief interventions, crisis management, and consultation to patients with behavioral health needs.

Key Responsibilities

You will monitor mental health referrals, coordinate care for complex patients, and act as a resource to the care team. Responsibilities include staff training, chronic care self-management support, documentation, and data collection. Requirements
