Key Responsibilities

• Conduct home visits in designated territories.
• Perform diagnostic tests and assessments.
• Develop and implement individualized care plans.
• Educate clients and families.

Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year recent clinical experience.
• Graduation from accredited Physical Therapy program.
• Ability to work independently.

• EMR experience preferred.
