BAYADA Home Health Care seeks an experienced Physical Therapist for full or part-time home health visits in Woodbridge, VA. You will serve adult and geriatric clients in their homes.

Key Responsibilities

• Conduct home visits and perform diagnostic tests.
• Develop and revise individualized care plans.
• Educate clients and families.
• Document care using EMR.

Requirements

• Current VA Physical Therapist license.
• Graduation from accredited program.
• Ability to work independently.
• Strong interpersonal and organizational skills.
• EMR experience preferred.
