About the Role

Join a new, innovative healthcare provider in Denver, Colorado as a Nurse Practitioner in primary care. This outpatient role focuses on seniors with complex chronic diseases in a value-based care model.

Key Responsibilities

• Provide primary care to seniors with 3-5 complex diseases.
• Manage a patient panel up to 650.
• Work Monday-Friday, 8am-5pm.
• Participate in a call schedule based on provider market size (low frequency).
• Collaborate with a dedicated care team including MA, scheduler, and referral specialist.

Requirements

• Nurse Practitioner or Physician Assistant license.
• Bilingual in Spanish preferred.
• 2+ years primary care outpatient experience.
• Current DEA registration.
• Proficient in office procedures like suturing and joint injections.

• Salary starting at $130,000 plus up to 10% bonus.
