About the Role

Join BAYADA Home Health Care as a Weekend Home Health Physical Therapist providing care to adult and geriatric patients in their homes around Worcester, Auburn, Shrewsbury, and nearby towns. This part-time or full-time flexible role focuses on skilled nursing and rehabilitative care to help clients maintain independence.

Key Responsibilities

• Conduct home visits to clients.
• Perform diagnostic tests including mobility, balance, strength, and safety assessments.
• Develop and implement individualized care plans.
• Collaborate with nursing and multidisciplinary teams.
• Educate clients and families on rehabilitative care.
• Document care management using tablet technology.

Requirements

• Current Physical Therapist license in Massachusetts.
• Minimum one year recent clinical experience as a licensed Physical Therapist.
