Pharmacist - Medication Therapy Management (MTM) at SGS Consulting (Remote)

About the Role

This is a remote Pharmacist position focused on Medication Therapy Management (MTM) using telephonic comprehensive medication reviews. You will work from home delivering clinical assessments and medication reconciliation for targeted members.

Key Responsibilities

• Perform telephonic comprehensive medication reviews including medication reconciliation and clinical assessments.
• Identify, document, and facilitate resolution of medication-related problems.
• Create patient-friendly written summaries of each interaction.
• Prepare faxed materials for prescriber offices to resolve medication issues.
• Build and maintain positive relationships with physicians, nurses, and internal customers.

Requirements

• Bachelor’s Degree in Pharmacy or PharmD from an accredited school.
• Active pharmacist license in HI, TX, IL, FL, or PA.
• Previous experience delivering MTM using Outcomes MTM Connect platform.
• Basic computer skills and proficiency with Microsoft Office.
• Strong organizational and interpersonal skills.
• Experience working from home and in call center environments.
