Key Responsibilities

• * Travel to patients’ homes to provide personalized physical therapy services that support recovery and independence.
• * Conduct thorough assessments to evaluate each patient’s condition and rehabilitation needs.
• * Develop and implement goal-oriented treatment plans in collaboration with patients and caregivers.
• * Professional Development: Ongoing training, mentorship opportunities, and support for career development.

Requirements

• * Unencumbered Florida Physical Therapy License (required).
• * Experience with OASIS documentation (preferred).
• * CPR Certification (preferred).
