About the Role

Join an innovative healthcare provider in Denver, Colorado as a Nurse Practitioner focused on primary care for seniors with complex chronic diseases. This outpatient role emphasizes value-based care in a supportive team environment.

Key Responsibilities

• Manage a patient panel of up to 650 seniors.
• See 12-14 patients per day.
• Work Monday through Friday, 8 am to 5 pm.
• Participate in a value-based care model.
• Collaborate with a dedicated care team including MAs and referral specialists.

Requirements

• Nurse Practitioner or Physician Assistant license.
• Bilingual in Spanish preferred.
• Minimum 2 years of primary care outpatient experience.
• Active, unrestricted license and DEA registration.
• Proficient in office procedures such as suturing and joint injections.
