About the Role

Join BAYADA Home Health Care as a Physical Therapist providing skilled rehabilitative care in patients' homes. You'll work with a multi-disciplinary team to help clients maintain independence.

Key Responsibilities

• Perform diagnostic tests and mobility assessments.
• Develop and implement individualized care plans.
• Educate clients and families on rehabilitative activities.
• Document patient care accurately using EMR.

Requirements

• Current Physical Therapist license in Arizona.
• Graduation from an accredited Physical Therapy program.
• Solid computer skills; EMR experience preferred.
