Dietitian (Clinical) - Home Based Primary Care (HBPC)

About the Role

Serve as a Clinical Dietitian providing Home Based Primary Care (HBPC) nutrition services within the Clinical Section of Nutrition and Food Services in Port Charlotte, Florida. This role involves autonomous application of evidence-based nutrition practice guidelines for patients with complex medical and nutritional needs.

Key Responsibilities

• Conduct comprehensive nutrition assessments using the Nutritional Care Process.
• Develop and implement Medical Nutrition Therapy tailored to individual patient needs.
• Document interventions and monitor outcomes using approved formats.
• Identify patients needing nutritional support and coordinate with healthcare teams.
• Participate in interdisciplinary team meetings.
• Provide tailored nutrition education and counseling to Veterans.
• Utilize virtual care technology to improve access.
• Maintain productivity and accurate clinical records.
• Lead implementation of new initiatives and quality improvement activities.

Requirements

• Valid driver's license with good driving record.
• Ability to travel extensively (76% or greater).
• Permanent appointment with promotion potential to GS-11.

Benefits & Compensation

• Salary range: $50,460 to $97,087 per year.
• GS 7-11 pay scale.
• No relocation expenses reimbursed.

Work Schedule

• Full-time position.
