About the Role

BAYADA Home Health Care is seeking a New Graduate Physical Therapist for full-time home health visits in Lebanon County, Pennsylvania. This role involves providing rehabilitative care to adult and geriatric clients in their homes as part of a multidisciplinary team.

Key Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, strength, balance, and safety assessments.
• Implement wound care interventions using evidence-based dressings.
• Develop and revise individualized care plans including therapeutic exercises and gait training.
• Educate clients and families on rehabilitative care and safety.
• Accurately document care using electronic medical records.

Requirements

• Current Pennsylvania Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Computer proficiency; EMR experience preferred.
