About the Role

Licensed Therapist position in Greensburg, Pennsylvania with Forge Health Careers. This is an outpatient mental health and substance use provider focused on high-quality, evidence-based care. The role involves in-person and telehealth services treating addiction and mental health conditions.

Key Responsibilities

• Facilitate outpatient and intensive outpatient programs for mental health and substance use disorders.
• Provide individual and family therapy and case management.
• Conduct assessments using DSM criteria and determine appropriate level of care.
• Maintain treatment plans and documentation.
• Coordinate discharge planning and continuing care.
• Participate in multidisciplinary teams and case management meetings.
• Provide crisis intervention and other duties as assigned.

Requirements

• Valid, unrestricted Pennsylvania license as LCSW or similar credentials (LSW, LAPC, LPC, LMFT).
• Master's degree in Social Work or related field.

Work Schedule
