Physical Therapist (PT) - Home Health Visits

About the Role

Join BAYADA Home Health Care as a Physical Therapist providing home health visits in Central New Jersey. This full-time role serves adult and geriatric clients in senior living communities and traditional home health settings.

Key Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• Educate clients and families on rehabilitative care.
• Document observations and interventions using electronic medical records.

Requirements

• Current New Jersey Physical Therapist license.
• Minimum one year recent PT experience.
• Graduation from an accredited PT program.
• Ability to work independently with strong organizational skills.
• Computer proficiency; EMR experience preferred.
