About the Role

Join BAYADA Home Health Care as a Physical Therapist performing home health visits in Lexington, South Carolina. You will provide rehabilitative care to adult and geriatric clients in their homes, helping them maintain independence.

Key Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions using electronic medical records.

Requirements

• Current South Carolina Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Computer proficiency; EMR experience preferred.
