Key Responsibilities
Conduct comprehensive health assessments via telehealth and document patient risk adjustments using ICD-10 and CPT II codes. Deliver clear care plans and follow-up guidance while closing HEDIS quality gaps for Medicare and Medicaid populations.
Requirements
Candidates must hold an active Iowa Nurse Practitioner license and have at least one year of experience in Family or Internal Medicine. Proficiency with Medicare/Medicaid populations and risk adjustment workflows is required.
