Key Responsibilities
The Nurse Practitioner manages a defined patient panel, performing clinical assessments and formulating longitudinal care plans for patients with chronic conditions. They collaborate with interdisciplinary teams and external providers to ensure high-quality care and reduce hospital readmissions.
Requirements
Candidates must hold a Master's degree in Nursing and possess at least 2 years of experience as a Nurse Practitioner in primary care, cardiology, or nephrology. A current state license, national board certification, and proficiency with Electronic Health Records are required.
