The Care Manager is responsible for the overall care management and quality of care for participants, coordinating holistic care plans in collaboration with an interdisciplinary team. They also provide supervision, coaching, and quality oversight for field staff to ensure adherence to clinical standards and regulatory requirements.
Candidates must be a board-certified Nurse Practitioner with a current, unrestricted license in New York State. The role requires at least two years of care planning experience and two years of administrative management experience in a healthcare setting.
